Five people have died after a truck tried to overtake two vehicles on the R33 near Doringfontein on the Vaalwater road on Thursday, Arrive Alive said in a statement.
"A Toyota Dyna truck tried to overtake two motor vehicles and collided head-on with an oncoming [Volkswagen] minibus. The Dyna truck overturned."
The 56-year-old driver of the truck and the four passengers of the minibus - two men and two women - were all declared dead.
The driver and four passengers of the minibus were seriously injured, the statement read.
SAPS Nylstroom is reportedly investigating the accident.
